Network Port Security Exemption Request

Features

By default, all wired network ports restrict access to a single device (defined as a unique MAC address, a.k.a. Ethernet address).  If you have a situation that requires the use of multiple MAC addresses behind a single network port, use this form to request an exemption from this policy and describe the reason for your request.  There are two types of exemptions:

Type A is for virtual hosts (VMWare, Hyper-V, etc.) or appliances that present multiple MAC addresses.

Type B is for connecting a switch to the OIT network. Typically, this exemption is used to allow workbench switches for configuring multiple computers. Up to 20 MAC addresses will be allowed.

All requests for exemption are carefully studied on a case-by-case basis and will be denied if an alternate solution exists.

Pricing

A one-time configuration fee of $50.00 will be charged to make each exemption change.

The monthly charge for an exempted port will be $30.00.
