IRIS: Updating the Beneficiary Designation Form

In accordance with Policy HR0307, the University of Tennessee provides benefits in the event of an employee death. Use the Beneficiary Designation Form to designate your beneficiaries who will receive payment for those benefits. You can submit a new Beneficiary Designation Form whenever information for your beneficiaries needs to be updated. To access the form, open the IRISWeb portal.
