Access Requests
This is a request to grant access to a computer, software, or network resource to an employee or department. Please include in the request:
- The NetID username of the person/persons who require access
- The resource(s) to be granted
- The person responsible for approvals in the department with the desired resource(s)
Computer Equipment or Software Purchases
This is a request for new, replacement, additional or updated equipment or software. Please include in the request:
- The cost center to be charged for the purchase
- The person within the department responsible for approvals for that cost center
- The desired recipient and location for installation of the equipment or software
Data or Software Information Requests
This is a request for service, or discussion of service, for software used principally by the Division of Student Life, including Anthology Engage, Innosoft Fusion, Maxient, Medicat and StarRez.
Temporary Equipment Requests
This is a request for short-term use of hardware, like scanners used to record event attendance or mobile hotspots. Please include in the request:
- The type and quantity of equipment desired
- The date, time and location for the equipment to be delivered
Please choose the "Onboarding" form if you need to start a request for computer setup and software access for a new hire.
Onboarding
This is a request to prepare a computer and grant access to software and network resources for a new employee. Info required for this request includes:
- Position number
- Position title
- Name
- NetID username
- Start date
- Whether early, sponsored access is required
