Lab Access Request

Please read these instructions in their entirety before you get started. You will need to save records as you complete some of these trainings.

1. Determine which lab spaces you will need access for. A list of labs can be found at: https://ne.utk.edu/safety/ and follow the link for “NE Departmental Training Matrix.”

2. Determine which trainings are needed to satisfy all of the labs you need access for.

3. Complete the trainings. Not all trainings are accessed in the same way. Most of the trainings can be found on EHS’s Canvas site, also linked at the above page.

Contact the Radiation Safety Department to register for the following:

            Radioactive Sealed Sources                           X-ray Machine

            Radioactive Open Sources                             Laser Safety

Please be aware that the Open Sources training is done in-person in a classroom setting, and will need to be scheduled with up to a few weeks of lead-time. The Radiation Safety Department will email training records for their courses directly to the NE Lab Safety Manager.

For UT employees, “Hazardous Waste & Chemical Spills” is hosted through K@TE.

If you cannot locate a required training, email the NE Lab Safety Manager at mratlif6@utk.edu.

4. Once you have completed all of the required trainings, organize your training records into a single digital file.

5. Complete the form, indicating the PI or Faculty Member you work for. If you are a PI, indicate yourself. Indicate all lab spaces that you need access for. Enter a brief description of your project or why you need access to these spaces.

6. Attach the file that you created in step 4.

7. Press the button at the bottom to submit your access request.

8. Access requests have the following routing order:

            A. Principal Investigator

            B. NE Lab Safety Manager

            C. UTK Access Management

9. You will receive system emails as these steps route through institutional approvers.
